What accommodations can I should I ask for? Afraid of getting rejected if I tell them what I feel I need

I have an anxiety disorder diagnosis from my family doctor and he has agreed to write a note to support my accommodations for the Jan test

I am just unsure what to ask for, what all I feel I need would be 53 min with no experimental section (not knowing which section counts paralyses me with anxeity, idk why), and a five minute break in between section

But I am afraid of getting rejected if I ask for that, my diagnosis is pretty recent and I haven't had accommodations in school in the past (mostly cause almost none of the work was timed in my program, mainly take home essays). Also even though my doctor recommend medication I have refused to take any.

For now i'm just planning on asking for the 53 min, unless someone who is familiar can give me a realistic view of what I can get
