Looking for a very old story

This one has to be more than 20 years old and all I recall are themes. There was a doctor who visited a guy and subjected his nuts to various forms of torture, week after week. One such visit involved a bunch of bees (or wasps/hornets?) stinging them. I think it ended in a ceremony with a bunch of robed people each doing something to the guy's nuts. Seem familiar to anyone?
